Beginning with the 2016-2017 school year: 
The school Board has adopted Policy 7:100 which requires all students entering grades Pre-K, Kindergarten, 6th and 9th grades to provide new and updated physicals and immunizations by the first day of school.

Students in grades 6 and 12 must also provide proof of immunization against meningococcal disease.

If the required documentation is not provided, your child will be excluded from school attendance beginning the first full day of school. 

Beginning with the 2017-2018 school year: The State of Illinois Public Act 099-0843 requires schools to request parents of students with asthma to provide an Asthma Action Plan from the student's healthcare provider.  This action plan will be kept on file with the school nurse. This Action Plan needs to be submitted at the start of each school year. 


All incoming 9th graders, new students transferring from an out of state school, and students planning to participate in sports will need a physical examination completed within the past year. Copies of the required physical examination form, ILLINOIS DEPT. OF HUMAN SERVICES, CERTIFICATE OF CHILD HEALTH EXAMINATION, are available from the school office, the nurse, or linked below. Parents/guardians please complete the Health History Section, sign and date it before seeing the health care provider. 


All high school students must show proof of completion of the Tetanus, Diphtheria, Pertussis, Polio, MMR, Varicella, and Hepatitis B.  

Additionally, 12th graders are required to show proof of Meningococcal immunizations. 2 doses are required with the first dose on or after the 10th birthday and the second dose at least eight weeks after the first dose and after the 16th birthday. Only one dose is required if the first dose was received at 16 years of age or older.

If any series is incomplete, a schedule for completion must be obtained from the clinic or doctor for your child to attend school.

Vision Exam Requirements

All students transferring into the Mascoutah School District from out of state are required to have a vision examination by a licensed Optometrist or Ophthalmologist. Click on the attachment below to obtain a copy of the State of Illinois Eye Examination Report form.

Only in the case of prescribed medication or treatment necessary for the student to remain in daily attendance or where failure to take medication or treatment could jeopardize the student's health, should medication or treatment be administered at school. Forms for requesting medication or treatment to be given at school (including prescription and over-the-counter medication) and forms for self-administration of asthma inhalers or Epi-pens are available from the school office, the health room, or online.
